Montessori Care (June 2023)

Description

Forest and Farm Montessori Care is for toddlers through children age 4 (pre-kindergarten). This registration form is for Monday June 5 – Friday June 30th, 2023. 

By enrolling in a full month of Forest and Farm Montessori Care at one time allows us to plan for supplies, activities, and staffing more accurately, so we offer a $200 discount (total value $1,400 for $1,200 which is only $300 per week!)

Upon purchase you will be able to download the registration form to fill out about your child and family and return to info@amadeusfarm.com before the start of camp. We will then call you to discuss meeting your child and scheduling a time for them to visit and get acquainted with us and the farm with their guardians present. The more information we receive and the earlier we receive it the easier it will be to make accommodations if needed. Please send us your information as soon as possible.

4. Camp Sessions

• Which session(s) would you like to register for? (Please indicate the date(s) and session name, if applicable)
• During all camp weeks,  we will provide a healthy and farm or locally sourced snack for your child in the morning and afternoon included in the cost of camp. What type of snacks and drinks does your child enjoy?
• Base camp fees do not include lunch except for the two Farm to Table food themed camps. Will your child bring a lunch or do you want lunch cooked for them at camp? Camp lunches are $20 per week in advance or $5 per day drop-in. What types of lunch foods does your child enjoy?

5. Transportation and Early/After Care

• How will the camper be arriving to and departing from the camp? Include a list of all people who may pick up your child and then general schedule for their pick up days.
• Will you be needing early drop off (before 8:45-9am) or late pick up (after 3:30-3:45)?

6. Additional Information

• Is there anything else you would like us to know about the camper (e.g. interests, hobbies, special needs)?
• Please describe your child’s sleep and eating cycles. Will your child nap at camp?
• For toddlers please provide other information about what makes them comfortable such as pacifier use, use of sign language, favorite toys and songs, diaper routine, etc.

7. Media Release

• Do you want photos of your child sent to you? If so please provide the best method of sharing photos directly.
• Do you grant us permission to use photographs or videos that show your child’s face during the camp for promotional purposes? (Yes/No)

8. Waiver and Release

• By signing this registration form, I acknowledge that I have read and agree to the camp’s policies and procedures, filled out the medical care and ambulance form, and I waive and release any claims against the camp and its staff for any injury, loss, or damage that may occur during the camp session.
